Combined upper and lower (thoracic and lumbar) spinal curves (double major curve) are usually equal curves, each containing 5 to 6 spinal bones. The upper curve usually bends to the right, and the lower curve usually bends to the left. This type of curve is not as deforming as a single curve because the trunk usually remains aligned (head centered over sacrum). Single major lower spinal (lumbar) curves are the least deforming of all curves, but they can cause the waistline to become distorted. They usually include 5 spinal bones. They account for about 1/4 of curves caused by scoliosis of unknown cause.

Single major curve that affects both the upper and lower spine (thoracolumbar curves) produces more cosmetic deformities than separate upper and lower curves of the same magnitude, especially when the curves are long. This type of curve usually contains 6 to 8 vertebrae and usually curves to the left 2. They account for less than 1/5 of curves caused by scoliosis of unknown cause 1.
